Why Low Libido Is Rarely Just One Thing
Low libido is one of the most common things patients bring up with me, and it's one of the most misunderstood. Women get told it's just stress, just age, or just menopause, and they get handed one prescription like a single hormone is going to fix it. I talked about this with Christa Biegler on The Less Stressed Life podcast, and I want to walk through it here too, because low libido is rarely just one thing.
It's Rarely Just a Hormone Problem
Stress hormones affect sex hormones. That's the piece people miss. Chronic stress can quiet your desire long before anything shows up as abnormal on a lab test. Add in poor sleep and not eating enough, which I see constantly in the high achieving women in my practice juggling a career and a full household, and your body starts shutting down anything it doesn't think it needs for survival. Libido is one of the first things to go.
What Testosterone Actually Does (and Doesn't Do)
Everyone wants to talk about testosterone, but the number I actually care about is free testosterone, not total. If your sex hormone binding globulin is high, which happens with insulin resistance or with oral contraceptives, that testosterone gets bound up. It shows up fine on paper, but your body can't use it. When I do treat someone, I'm looking for serum testosterone around 50 to 100 ng/dL and free testosterone around 3 pg/mL, and I pick the delivery method (cream, patch, injection, or pellet) based on what that patient will actually stick with.
And more is not better. Too much testosterone can cause acne, new facial hair, or thinning hair on the scalp through the DHT pathway. That's exactly why I individualize and monitor dosing instead of maxing it out.

Where I Start
Before I touch a single hormone, I want the foundation in place. Are you managing stress? Are you sleeping? Are you eating enough? Is your blood sugar stable? Even how you metabolize estrogen depends partly on an enzyme called COMT, and stress impairs it. So if I skip the foundation and jump straight to a prescription, I'm treating a symptom while the real driver keeps working against us.
