GLP-1s in Midlife: Where They Fit Alongside Hormone Therapy
On The Optimal Body Podcast, physical therapists Doc Jen and Doctor Dom and I talked about one of the biggest questions in women's health right now: where do GLP-1 medications fit for women in perimenopause and menopause?
Why Midlife Weight Feels Different
As estrogen declines, many women notice weight shifting to the midsection, more insulin resistance and less muscle, even when their habits haven't changed. It's not a willpower problem. It's physiology.
GLP-1s Can Help, With Guardrails
GLP-1 medications can be a valuable tool for appetite and blood sugar. But in midlife, protecting muscle and bone is essential, and rapid weight loss can cost both. If you're using a GLP-1, make these non-negotiable:
Adequate protein at every meal
Strength training several times a week
Hydration and enough fiber for digestion
Regular check-ins with a provider who monitors more than the scale
Hormones Are Part of the Picture
For many women, addressing estrogen, progesterone and sometimes testosterone improves sleep, energy, recovery and body composition. GLP-1s and hormone therapy aren't competing options. For the right patient, they can work well together as part of an individualized plan.
The goal is long-term health and strength, not just a smaller number.
